CHANGING TRENDS OF THE PEPTIC ULCER DISEASE

Abstract
To determine changing trends in surgical aspect of the peptic ulcer disease possibly influenced by the widespread usage of the Histamine H2-receptor antagonists and technical improvement of the fiberoptic endoscopy, we reviewed 1,497 cases of peptic ulcer disease who were undergone operations at Department of Surgery, Korea University Hospital and a 30 years of the study periods, from January 1959 to December 1988, were subdivided

into three by the decade. The results were as follows

1. The incidence of duodenal ulcer was increased greatly, and the proportion of the emergency operation among total number of the peptic ulcer patient underwent operative therapy increased in the recent period.

2. The number of emergency operation was increased after the clinical application H2receptor antagonists.

3. Patients with acute ulcer, defined as those with symptoms of less than three months duration, was increased after the clinical application of the histamine H2-receptor antagonist

4. In period III, the most common operative procedures in gastric ulcer is subtotal gastrectomy and that in duodenal ulcer is a truncal vagotomy with pyloroplasty The peptic ulcer is one of most common gastrointestinal disorder in Koera as well as in western countries. At the same time, it is a field of the disease entity even though most studied still controversy present. Since mid 1970¢¥s, the advent of the histamine 12 Receptor antagonist and it¢¥s clinical application with accompanied technical refinement of diagnostic upper gastrointestinal endoscopy resulted significant changes in clinical aspect of the peptic ulcer disease.
